Dixon Soccer Club will be holding Metro Soccer Registration

Metro Registration Fee $125.00

Following items are REQUIRED with your Registration:
• Registration Fee (Cash, checks or money orders payable to DSC) $25.00 service charge on all returned checks
• Current Photo cut to 1x1 (Print name and birth date on back)
• Copy of Certified COUNTY Birth Certificate

If not complete with ALL items they will be RETURNED.
