Dixon Soccer Club will be holding soccer registration
for the up coming season. Please see the following
dates, times and locations.

Registration Dates Fee $100.00
May 17th from 10:00 AM to 2:00 PM
Lincoln Medical Plaza, Conference Room
Late Registration
After May 31st Registration will be $120.00
Registration cut off date is June 15th
Bring the following items with you to
Registration:
- Registration Fee (Cash, checks or money orders
payable to DSC) $25.00 service charge on all
returned checks
- Current Photo cut to 1x1 (Print name and birth
date on back)
- Copy of Certified COUNTY Birth Certificate
- SYSL policy change this year ALL players must
turn in Certified COUNTY Birth Certificate. This
WILL NOT be returned to you.
- A child must be 4 years old by July 31, 2008 in
order to play
Drop Off Location – 1425 Market Lane, Suite F,
Dixon
Starting April 28th during business hours registration
forms will be available for pick up and drop off.
If packets are not complete with
ALL items they will be RETURNED to you via
mail.
