Please drop off all COMPLETE registration packets at
DREAM HOME REALTY located at 130 West A Street (Map),
Dixon during normal business hours. PLEASE MAKE SURE
YOU INCLUDE A 1X1 PHOTO (NO HATS OR SUNGLASSES), A COPY
OF YOUR CHILDS BIRTH CERTIFICATE AND SIGN THE
APPLICATION.
LATE REGISTRATION FEE
IS $105.00. PLEASE NOTE LATE REGISTRATIONS WILL BE
PLACED ON A WAITING LIST AND ARE NOT GUARANTEED
PLACEMENT ON A TEAM.
NO REGISTRATIONS WILL BE
ACCEPTED AFTER JULY 1ST!
